Consulting Group, Inc. All contracts previously executed under the unincorporated
name of"HCG" have been transferred to and will be honored under the new corporation
name of"Hilts Consulting Group, Inc."
All ' cece the January1, 201 ' , effective date has been transmitted under
the new Hilts Consulting Group, Inc. letterhead, and as such, all income should be
reported under the new corporate structure. Please forward this letter and the enclosed
signed original W-9 to your accounting department for proper processing, reporting of
income, and issuance of year-end statements.
